For example, by having lunch every month or eating together at a family table in a restaurant in the neighborhood. We work intensively with general practitioners, home care workers, elderly and district nurses and together ensure that the forgotten group of independently living elderly people return home and thus increase their social network. Our volunteers take care of the contacts with these care partners and then invite the elderly, provide transport and are hostess to the meetings. Of course we would like to tell you more about how we work, what our approach is and how to start with your heart in Hoorn.
