About PxZorg

PxZorg: is a vision of how we want to organize health and care together in North Limburg. PxZorg is described in the 5 principles:
• Prevention in the Population: healthy living and avoiding risks, reduce the risk of disease.
• Patient as a Person: Every person is unique and personally responsible for his or her health, under their own direction and control. The professional supports the care recipient as a partner in (re) finding health.
• Participation as a Priority: no new care without involving care recipients in the development.
• Paths with Partners: Care paths and chain care in which partners work well together across the various lines.
• Professional Performance: healthcare professionals work according to established standards and are prepared to render (peer) accountability.

As PxZorg, we believe that the voice of the citizen in the policy, projects and developments of VieCuri (hospital), Cohesie (general practitioners cooperative) and other parties involved in transmural care, also called chain care (e.g. mental health care, municipality, home care etc.) should be heard.

To ensure this, PxZorg has a patient participation team. This team consists of a citizen representative, a citizen participation advisor from Burgerkracht Limburg and the coordinator PxZorg who works for Cohesie and VieCuri
